New Students - By the FIRST DAY OF SCHOOL, all new students in the State of Hawaiʻi MUST HAVE:

1. Tuberculosis (TB) Clearance

2. A completed Student Health Record (Form 14) including a physical examination and all required immunizations OR a signed statement or appointment card from the childʻs medical doctor.  (Information from the Hawaiʻi Department of Health)
