Enrolling in the DOE for the 1st Time
The following information is needed for children enrolling in the public school of Hawaiʻi for the first time:
Original copy of the birth certificate or equivalent (passport) - no copies or pictures: For age verification. Student passport or student visa is acceptable for foreign students.
Documentation that a Tuberculin (Mantoux) test was done within 12 months before school entrance; A student will not be allowed to enter school unless this requirement has been met. This is a requirement from the Department of Health.
Form 14 or its equivalent, on which a recorded current physical examination was done within a year from entry, a tuberculin test clearance, and updated immunizations
Proof of Residence: Please follow the guidelines carefully
Utility bill, lease agreement, or other official documents showing your current address.
A notarized statement by a relative/friend can be accepted by the school with the following stipulation:
(a) Notarized statement must state that the parent/legal guardian and child are living with the relative/friend;
(b) Notarized statement must state the name of the relative/friend that is on the relative/friend's proof of legal residence;
(c) Notarized statement must state the same address of relative/friend that us on the relative/friend's proof of legal residence;
(d) A copy of the relative/friend's proof of legal residence must be attached to the notarized statement; and
(e) Notarized statement must be signed by the same name of relative/friend that is on the relative/friend's proof of legal residence.
Renter: Current Rental/Lease Agreement
Valid photo ID of parent/guardian

FAMILIES IN UNSTABLE HOUSING: Families experiencing unstable housing are covered by enrollment guidelines provided in the McKinney-Vento Act. Please contact a community liaison in your area.
Falsification of documents submitted is subject to penalty under Hawai'i Revised Statutes 710-1063, resulting in the child being sent back to the school where he/she should properly be attending. The Department may pursue prosecution at its discretion.
Kindergarten Registration
A student must be 5 years old on or before July 31 to enter Kindergarten. For more information, click here
Students Transferring from another Local School:
Students transferring to Pūʻōhala Elementary School from another Hawaiʻi DOE school must present a release card (F211) which is issued by the releasing school and their proof of residency (please follow the guidelines listed above). Students transferring from an out-of-state school should, if available, present a release card to facilitate registration and placement. Students entering Ke Kula Kaiapuni ʻo Pūʻōhala are NOT required to complete Geographic Exception.
Students Transferring from a Hawaiʻi Private School or Out-of-State
A release card (F211) is not needed but will be required at the end of the current school year. Proof of residency (please follow guidelines listed above) or a legal document that verifies you have purchased a home in Pūʻōhalaʻs home school area is needed. Pūʻōhala School will notify the pupilʻs current Hawaii DOE school of the registration. Students entering Ke Kula Kaiapuni ʻo Pūʻōhala are NOT required to complete Geographic Exception.
Release Packet or Withdraw Documents From Former School
Transferring? Please notify the schoolʻs front office, either by phone or written notice, as soon as you know that you will be moving out of our school district. This will expedite the process in preparing the proper paperwork for transferring your child.
Updating Student Records
Please notify the school whenever there are changes to your address and/or phone numbers. For your childʻs welfare, it is vital that this data be current so that you can be contacted in case of an emergency.
